Is the address proof required for applying OHIP?

I would like to ask if the address proof is a mandatory document for applying OHIP. Can it be provided later?

Moreover, the validity period is generally 5 years?

Can I use the address in BC for applying that?
Once I get a permanent address, I will update the record.
