In-Person Health Card Application - Work Permit

Good morning, I would like to know to which specific Service Canada location we have to go to apply in person for a Health Card in the city of Toronto (we are living downtown). I have a 2 year work permit and my wife has one too.

You should get an application form online or from your local Service Ontario OHIP Office.

Use the ServiceOntario Service Finder to find the closest one to where you are located.

You will see that it will pull up a listing of offices. In each listed office, it will indicate which services are provided at the office.

Make sure that the office you select that is closest to you provides services related to OHIP.

Hi Anna,

This isn’t correct. If the person seeking an OHIP card is not a PR, they have to go to 777 Bay St. in Toronto. Found out the hard way after going to the one by Trinity Bellwoods and waiting in line for 15 minutes.
