Documents Required for Schooling

My kids are in primary and secondary (Years 4, 5, and 7). I would like to know the documents required to enroll them in school in Ontario.

Particularly, is a transcript issued from their last school required, or will copies of report cards suffice?

Will they need to take entrance exams?
