Add a Name or Legal Name Change?

Hi, I need some advice.

I am 18 years old turning 19 in February 2015.

On my birth certificate, my name is in this format: Engligh name, Chinese name, family name.

However, growing up I was never called by my English name on my birth certificate because my mother decided to use another name for me.

Whenever I signed up for school or activities I would be called by Matthew, although not indicated on my birth certificate.

Now, for legal and registration purposes, I wish to have Matthew added so I can use it legally.

I don’t know what procedures to take to have this name added.

Would this be a legal name change or an amendment?

Also, who do I contact for this procedure to be done?
