Question on section H - Date of Issue?

What does “Date of Issue” mean in regards of this section?

Thank you for sharing your question with us.

As you may already know, you should fill out this section only if you are applying to replace a lost, stolen, destroyed or never received Permanent Resident Card.

“Date of Issue” refers to the date your lost,stolen, destroyed or never received Permanent Resident Card was issued.

PR cards are valid for a period of five years from the date of issue.
