Eligibility for OHIP - Questions

My wife has an open work permit and has been working for 4 months. What should we do to get OHIP.

will the last 4 months be considered because I read at your website that we need an agreement to show that my wife will be working for at least 6 consecutive month.

I wife is a Canadian citizen and I am a US citizen with permanent resident status. I understand that we may lose OHIP benefits after 6 months if we move to the USA. What is necessary to regain OHIP benefits if we return to Canada?
